What is professional documentation?
- Professional documentation is documentation that is provided by a professional licensed to diagnose the particular disability
- Professional documentation verifying disability and explaining limitations must be provided initially to help determine eligibility for DSS at Dalton State. Additional documentation may be required during the eligibility process and/or periodically during the student's time at Dalton State

All students must provide documentation that meets BOTH general and specific guidelines. Please select from the diagnoses below, and read to ensure all requirements are met. Print the specific documentation and provide to your doctor, if necessary.

Learning Disabilities
Acquired Brain Impairment
Visual/Hearing/Motor Impairment
ADD/ADHD
Psychiatric/Psychological Impairment
Chronic Illness
- Most recent documentation within three years
- Must be comprehensive
- Must provide processing information identified from testing other than those used to obtain the global IQ score
- Must be within 1 year or specify stability of impairment
- Must contain medical history
- Must provide detailed cognitive deficits/abilities
Visual/Hearing/Motor Impairment ![]()
- Must provide diagnosis and limitations
- Must specify stability of impairment or be current
- Must submit completed packet provided by DSS or provide developmental history in writing from multiple settings
- Must provide current symptoms • Must rule out psychiatric or medical disorders that cause attention problems
Psychiatric/Psychological Impairment ![]()
- Must submit completed packet provided by DSS or provide evaluation and diagnosis
- Must be within 1 year or specify stability of impairment
- Must be within 1 year or specify stability of impairment
- Must provide diagnosis and limitations
