Immunization Requirements
The Board of Regents (BOR) of the University System of Georgia has established immunization requirements for all new students (first-year, transfers, and others). These requirements are outlined below and are required prior to registration at any college or university within the University System. Required Immunizations
- Measles, Mumps, and Rubella (MMR)
- Varicella (Chicken Pox)
- Tetanus, Diphtheria, Pertussis (Tdap)
- Hepatitis B (18 years and under)
Certificate of Immunization
All students must submit the following form to document proof of required immunizations. No other documentation will be accepted.

Submitting your Certificate of Immunization
Option 1:
Log in to your Nest Portal account
Click on “Choose File.”
Click “Select File” to attach your Certificate of Immunization and any required documentation.
Click “Upload.”
Option 2:
- Email your submission to [email protected]
- Fax your submission to 706-272-2530
- Mail your submission to the Office of Admissions, 650 College Drive, Dalton, GA 30720
Exemption Requests
Exemption Requests
Some students may qualify for an exemption from the College’s immunization requirements.
Religious exemption. Students requesting a religious exemption should submit a notarized affidavit (no older than 6 months and signed by the student or parent/guardian if the student is a minor).
Medical exemption. Students requesting a medical exemption should submit a verification letter from a healthcare provider that details that the student cannot be immunized because of medical reasons and documents whether the exemption is temporary or permanent.
Online student waiver. Students who are enrolled in a fully online program may request a review of their account and a waiver of the immunization requirements.
Note: In the event of a disease outbreak, students receiving an exemption from the immunization requirements may be excluded from College facilities.
Anyone who hasn’t submitted all required records and forms prior to starting classes at Dalton State College will have a hold placed on their account and will not be able to register for any future classes until the required documentation has been received. The student may also be restricted from access to other services.
